Inpatient Care Manager (RN)- Per-Diem, Days, 8 AM - 4 PM, Atlantic Health Overlook Medical Center
The Inpatient Care Manager plays a critical role in coordinating patient care across the full continuum of illness. This position partners closely with interdisciplinary teams to ensure effective utilization of healthcare resources, proactive care planning, and timely, safe discharges. By conducting daily clinical reviews, leading interdisciplinary rounds, and addressing barriers to care, the Inpatient Care Manager helps optimize patient outcomes while supporting quality, efficiency, and continuity of care.
