Care Coordinator- Inspir (UES)/Telehealth/On-Call

New York, New York April 19, 2026 Full Time

Care Coordinator – Telehealth, On-Call & Inspir

Location: Upper East Side, NYC (Inspir Clinic) | Hybrid potential
Company: Spear Physical and Occupational Therapy

About the Role

Spear Physical and Occupational Therapy is seeking a highly organized, patient-focused Care Coordinator to support three of our most dynamic service lines: Telehealth, On-Call (in-home care), and our Inspir clinic within a senior living community.

This is an ideal opportunity for someone who thrives in a fast-paced environment, enjoys problem-solving, and is passionate about delivering an exceptional patient experience.

What Makes This Role Unique

  • Exposure to multiple care delivery models: virtual, in-home, and in-clinic
  • A central role that directly impacts patient access and experience across services
  • Opportunity to work with diverse patient populations, including seniors in a residential setting
  • Hybrid work flexibility (in-person + remote days)

Schedule & Location

  • Primarily based at our Inspir clinic on the Upper East Side
  • Hybrid schedule with potential remote workdays depending on business needs

Why Join Spear

At Spear, we are committed to delivering life-changing care through both innovation and human connection. Our Care Coordinators play a critical role in that mission—serving as the first and ongoing point of contact for our patients.
