Spear Physical and Occupational Therapy is seeking a highly organized, patient-focused Care Coordinator to support three of our most dynamic service lines: Telehealth, On-Call (in-home care), and our Inspir clinic within a senior living community.
Unlike a traditional front desk role tied to a single clinic, this position serves as a central coordination hub, ensuring patients across multiple care settings receive seamless, timely access to therapy services. This is an ideal opportunity for someone who thrives in a fast-paced environment, enjoys problem-solving, and is passionate about delivering an exceptional patient experience.

What You’ll Do
- Coordinate and manage patient scheduling across Telehealth, On-Call, and Inspire services
- Serve as the primary point of contact for patient inquiries via phone, email, and online platforms
- Facilitate urgent and same-day Telehealth appointment requests
- Schedule in-home (On-Call) visits and communicate logistics with clinicians and patients
- Support scheduling and front desk operations for the Inspir clinic
- Collect copays and manage patient financial transactions accurately
- Communicate coverage details to Telehealth patients
- Maintain accurate patient records and documentation in EMR systems
- Collaborate closely with clinicians and internal teams to ensure continuity of care
- Troubleshoot scheduling conflicts and provide real-time solutions
What We’re Looking For
- 1–3 years of experience in a healthcare administrative, front desk, or care coordination role preferred
- Strong organizational skills with the ability to manage multiple workflows simultaneously
- Excellent communication and interpersonal skills
- Comfort working with technology, including EMR systems and scheduling platforms
- Ability to adapt quickly in a dynamic, patient-centered environment
- High attention to detail and accuracy, especially with scheduling and payments
- A proactive, solutions-oriented mindset
