Job Description.

Works under the supervision of the DOO/CM and provides comprehensive case management to patients and their families. Develops, implements, coordinates and evaluates patient care plans according to physician orders as allowed by their scope of practice.

SKILLS AND CERTIFICATIONS

Home Health Experience

Recent Psychiatric experience in an acute hospital or in home health

IDEAL CANDIDATE

RN with current psychiatric home health experience and will travel valley wide (Surprise, Phoenix, and Chandler).
