Current licensed as a Physician Assistant or Nurse Practitioner in the State of Illinois;

Bachelor's Degree Program from an accredited Physician Assistant or Nurse Practitioner School;

Current CPR certification. Professional competencies documented by references based on Physician Assistant or Nurse Practitioner experience, education or both.
