POSITION SUMMARY
As an MDS Social Worker, you will provide assessment services, ensure compliance with documentation requirements, and function as a member of the interdisciplinary team. You will support patient care through completion of MDS assessments and collaboration with clinical teams, while also serving as a backup to Transitional Care Coordinators within the Jewish Home & Rehab Center short-term rehabilitation units.

KEY RESPONSIBILITIES
• Complete Minimum Data Set (MDS) assessments in accordance with CMS guidelines.
• Support Transitional Care Coordinators in discharge planning, acting as a liaison between patients, families, internal departments, and community referral partners.
• Assist patients and families in accessing concrete services and resources.
• Respond to crisis situations involving patients, families, and staff.
• Provide referrals for individual counseling services as part of the assessment process.
• Participate in the development of patient care plans, including establishing goals and treatment interventions for social services.
• Maintain accurate and timely documentation in compliance with federal and state regulations and organizational policies.
• Maintain strict confidentiality of patient and organizational information.
• Perform other duties as assigned.

• Pay Rate: $30 per hour
• Hours: Full-Time, 40 hours per week
• Shift Schedule: Day
• Days Off: Saturday & Sunday
• Working Days: Monday - Friday

REQUIRED QUALIFICATIONS
• Bachelor’s degree in Social Work, Psychology, or a related field required.
• Demonstrated ability to handle sensitive and confidential situations with professionalism.
• Strong interpersonal skills with the ability to build rapport quickly with patients, families, and team members.
• Excellent customer service skills.
• Strong written and verbal communication skills.
• Ability to read, write, and comprehend verbal and written instructions in English.
• Detail-oriented with the ability to make independent decisions and maintain accuracy.

PHYSICAL REQUIREMENTS
• Ability to lift, push, or pull a minimum of twenty-five (25) pounds.
• Ability to exert muscle force to lift, push, pull, or carry.
• Ability to use abdominal and lower back muscles over time without fatigue.
• Ability to stand and/or sit for extended periods.
• Ability to bend, stoop, stretch, twist, sit, and reach.
• Fine motor skills with good visual and auditory acuity.
