Private Duty Nurse (RN/LPN) – 1:1 Home Care
Job Summary
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
Client Case Details
📍Milford, MI
- Client Info: This is a sweet 3 month old who has a history of a congenital diaphragmatic hernia, VA ECMO, pulmonary edema, and feeding intolerance.
- Clinical Needs: Continued high-acuity care, including oxygen and NJ tube feeding.
- Environment: Clean pet-free home
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
Schedule & Pay
- Full-time and part-time shifts available
- Day, night, and weekend shifts
- Weekly pay + optional daily pay

Qualifications
- Active RN or LPN license in the state of Michigan
- Home care or private duty experience preferred (not required)
- Trach/vent experience a plus (training provided)
- Strong communication and critical thinking skills
