Licensed Social Worker Case Manager supporting Medicaid SMI & Substance Abuse Population in Utah, working remotely and participating in interdisciplinary care team meetings to ensure members have care plans based on their concerns/health needs.

Requirements

2 years health care experience, preferably in behavioral health
Licensed behavioral health clinician (LCSW, APSW, CHES, LPC, LPCC, LMFT, PhD, or PsyD)
Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance
Experience with working with persons with severe and persistent mental health concerns and serious emotional disturbances, to include substance use disorder and foster care
