About the Opportunity

We're looking for a Patient Navigator Intern to support our Health-Related Social Needs (HRSN) care navigation program. Working alongside clinical and operations teams, you'll help patients overcome real-world barriers - housing instability, food insecurity, transportation gaps - that stand between them and consistent, quality care. This role functions similarly to a case manager and is an ideal fit for someone pursuing a career in community mental health, social work, public health, or healthcare operations.

What You’ll Do

  • Screen patients for health-related social needs: Conduct structured assessments using established screening tools and internal workflows.
