The Opportunity

Town Square Health is hiring a Primary Care Provider (MD/DO) in the Rochester area to deliver high-quality primary care in a Medicare-focused, value-based care environment. This is a clinic-based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40-minute visit lengths based on patient needs, and a team-based care model with two Medical Assistants paired with each provider.

You’ll play a key role in delivering excellent clinical care, coordinating across the care team, and helping shape a new care model designed to improve outcomes and patient experience.

Qualifications

  • M.D. or D.O. degree
  • Board-certified in Internal Medicine, Family Medicine, or Geriatrics
  • Active New York State Medical licensure in good standing, or must obtain prior to start date
  • Applied for DEA registration prior to hire/start date
  • Must meet all applicable employer and payer credentialing requirements
  • Ability to meet post-offer requirements, including drug screen and health assessment
