As a Care Navigator, you’ll be part of a growing team that’s changing the way people experience care—providing telehealth-based support that bridges the gap between healthcare and everyday life. How You’ll Make an Impact:

  • Perform assessments and develop personalized care plans that address medical, emotional, and social needs

What You Bring:

  • Active LCSW license in good standing

  • Specialization in care coordination, case management, or clinical advocacy—preferably serving Medicare or older adult populations. preference for individuals with demonstrated experience navigating care for individuals with complex and acute medical and behavioral health conditions

  • A deep understanding of Medicare, social drivers of health, and complex care systems

  • Exceptional communication and interpersonal skills, with a passion for serving aging adults and their caregivers

  • Experience and comfort with using technology and telehealth platforms, and a readiness to adapt in a fast-evolving care environment
